- The death claim MUST be submitted through the policyholder (i.e. the employer of the insured) or the Human Resources Department of the policyholder.
- For Death Certificate or documents not written in English / Chinese, the Claimant needs to provide the certified valid translation
- If for any reason the original of any document is not available, a copy certified by a notary public should be provided
- Certified true copy means a photocopy endorsed with the company chop and signature by the policyholder.
- Generali may require additional documents
HOW TO FILE A CLAIM
Please report the death claim to your Company HR Department and get ready the below documents as soon as possible
SUPPORTING DOCUMENT(S) CHECKLIST
- ID Card copy of the deceased
- Certified true copy of death certificate
- Certified true copy of police report and hospital certificate (for unnatural death)
- Sick leave certificate and attendance records of the deceased
- Authorization for obtaining medical report
- Certified true copy of ID card of the beneficiary
- Proof of deregistration of ID card issued by the Immigration Department (if the deceased was a HKID cardholder and the death took place outside Hong Kong)
- Certified true copy of Notarial Certificate of Death, plus police report and hospital certificate (if the death took place outside Hong Kong)
- Certified true copy of the record of beneficiary designation, or Letter of Administration if the policy has no designated beneficiary and there is no will left, or probate if there is a will left
- Certified true copy of address proof of beneficiary/administrator issued within the last three months (e.g. gas, water, electricity, phone bill for land line or bank statement)
- Relationship proof copy between the life insured and the beneficiary
